with the hepatitis A virus (HAV)
and hepatitis B virus (HBV). Persons
with chronic liver disease are currently
recommended by the Advisory Committee
on Immunization Practices to
receive the hepatitis A (HepA) and
hepatitis B (HepB) vaccines if they
have not previously been vaccinated.
Recently, the Advisory Committee on
Immunization Practices approved
language clarifying that all patients
with hepatitis C virus (HCV) infection
are recommended for HepB vaccination1
and that persons with HBV
and HCV infections should also be
specifically considered for vaccination
against HAV

Anti-neutrophil extracellular trap antibody in a patient with relapse of anti-neutrophil cytoplasmic antibody-associated vasculitis: a case report

Neutrophil extracellular traps (NETs) are web-like DNA decorated with antimicrobial proteins, such
as myeloperoxidase (MPO), which are extruded from activated neutrophils. Although NETs are essential in innate
immunity, an excessive formation of NETs has adverse effects, e.g., induction of anti-neutrophil cytoplasmic
antibody (ANCA), to the hosts. Since ANCA can induce NET formation in the primed neutrophils, a positive
feedback loop can be formed between NETs and ANCA, which is called “ANCA-NETs vicious cycle.”

Genome-Based Analysis of Enterococcus faecium Bacteremia Associated with Recurrent and Mixed-Strain Infection

Vancomycin-resistant Enterococcus faecium (VREfm) bloodstream infections
are associated with high recurrence rates. This study used genome sequencing to accurately
distinguish the frequency of relapse and reinfection in patients with recurrent E.
faecium bacteremia and to investigate strain relatedness in patients with apparent
VREfm and vancomycin-susceptible E. faecium (VSEfm) mixed infection. A retrospective
study was performed at the Cambridge University Hospitals NHS Foundation Trust (CUH)
between November 2006 and December 2012. We analyzed the genomes of 44 E. faecium
isolates from 21 patients (26 VREfm isolates from 12 patients with recurrent bacteremia
and 18 isolates from 9 patients with putative VREfm/VSEfm mixed infection). Phenotypic
antibiotic susceptibility was determined using a Vitek2 instrument. Genomes
were compared with those of a further 263 E. faecium isolates associated with bacteremia
in patients at CUH over the same time period. Pairwise comparison of core genomes
indicated that 10 (71%) episodes of recurrent VREfm bacteremia were due to reinfection
with a new strain, with reinfection being more likely with increasing time
between the two positive cultures. The majority (78%) of patients with a mixed VREfm
and VSEfm infection had unrelated strains. More than half (59%) of study isolates were
closely related to another isolate associated with bacteremia from CUH. This included
60% of isolates associated with reinfection, indicating acquisition in the hospital. This
study provides the first high-resolution insights into recurrence and mixed infection by
E. faecium and demonstrates that reinfection with a new strain, often acquired from the
hospital, is a driver of recurrence

In the dawn of the laboratory medicine era, we
had a different view of quality. Back when we were
pipetting tests by mouth, and we were constantly
tinkering with the instrumentation, and the patient
volumes were, by today’s standards, miniscule,
we didn’t mind as much error. Error rates of
1%, 5%, 10%, that wasn’t so bad.
In the last half century, so much has changed, not
only in the laboratory but in the expectations of
quality management. What was once acceptable
in terms of defect rates has now become unsustainable
for today’s automated high volume operations.
When many laboratories are routinely
reaching millions of reportables per year (some
even per month!), even a 1% error rate is ruinous to
the laboratory’s ability to produce reliable turnaround
times and accurate results.

The pace of technical advancement in microbial genomics has
been breathtaking. Since 1995, when the first complete genome sequence of a
free-living organism, Haemophilus influenzae, was published,1 1554 complete
bacterial genome sequences (the majority of which are from pathogens) and 112 complete
archaeal genome sequences have been determined, and more than 4800 and
90, respectively, are in progress.2 A total of 41 complete eukaryotic genome sequences
have been determined (19 from fungi), and more than 1100 are in progress. Complete
reference genome sequences are available for 2675 viral species, and for some
of these species, a large number of strains have been completely sequenced. Nearly
40,000 strains of influenza virus3 and more than 300,000 strains of human immunodeficiency
virus (HIV) type 1 have been partially sequenced.4 However, the selection
of microbes and viruses for genome sequencing is heavily biased toward the tiny minority
that are amenable to cultivation in the laboratory, numerically dominant in
particular habitats of interest (e.g., the human body), and associated with disease.
In 2006, investigators reported in-depth metagenomic sequence data from a human
mixed microbial community5; in 2007 more than 1000 genes from single cells
of cultivation-resistant bacteria were identified.6 Since then, a flood of such data has
ensued (Fig. 1).7-9 Individual investigators can now produce a draft sequence of a
bacterial genome containing 4 million base pairs in about a day.10-12 The revolution
in DNA-sequencing technology has to a large extent democratized microbial genomics
and altered the way infectious diseases are studied.11 However, gene annotation
and error correction still take time and effort. Today, the major challenges in
microbial genomics are to predict the function of gene products and the behavior of
organisms and communities from their sequences and to use genomic data to develop
improved tools for managing infectious diseases.

Development of Envelope Protein Antigens To Serologically Differentiate Zika Virus Infection from Dengue Virus Infection

Zika virus (ZIKV) is an emerging flavivirus that can cause birth defects
and neurologic complications. Molecular tests are effective for diagnosing acute ZIKV
infection, although the majority of infections produce no symptoms at all or present
after the narrow window in which molecular diagnostics are dependable. Serology is
a reliable method for detecting infections after the viremic period; however, most
serological assays have limited specificity due to cross-reactive antibodies elicited by
flavivirus infections. Since ZIKV and dengue virus (DENV) widely cocirculate, distinguishing
ZIKV infection from DENV infection is particularly important for diagnosing
individual cases or for surveillance to coordinate public health responses. Flaviviruses
also elicit type-specific antibodies directed to non-cross-reactive epitopes of the infecting
virus; such epitopes are attractive targets for the design of antigens for development
of serological tests with greater specificity. Guided by comparative
epitope modeling of the ZIKV envelope protein, we designed two recombinant antigens
displaying unique antigenic regions on domain I (Z-EDI) and domain III (Z-EDIII)
of the ZIKV envelope protein. Both the Z-EDI and Z-EDIII antigens consistently detected
ZIKV-specific IgG in ZIKV-immune sera but not cross-reactive IgG in DENVimmune
sera in late convalescence (12 weeks postinfection). In contrast, during
early convalescence (2 to 12 weeks postinfection), secondary DENV-immune sera
and some primary DENV-immune sera cross-reacted with the Z-EDI and Z-EDIII antigens.
Analysis of sequential samples from DENV-immune individuals demonstrated
that Z-EDIII cross-reactivity peaked in early convalescence and declined steeply over
time. The Z-EDIII antigen has much potential as a diagnostic antigen for populationlevel
surveillance and for detecting past infections in patients.

Expected long-term defect rate of analytical performance in the medical laboratory: Assured Sigma versus observed Sigma

Reliability of laboratory results is determined by the ratio of incorrect results expected in long-term. Sigma is a measure of defect ratio, therefore
long-term Sigma is a measure of the reliability of laboratory results. Commonly, long-term Sigma is estimated based on the short-term Sigma. The
Six Sigma methodology assumes that in long-term performances will shift up to 1.5 Sigma, and therefore the long-term Sigma is considered 1.5 Sigma
less than short-term Sigma. Analytical performance in the medical laboratory is prone to shifts larger than 1.5 Sigma. Thus, the 1.5 Sigma shift
assumed in the Six Sigma is not a correct estimate in the medical laboratory. On the other hand, in the medical laboratory statistical quality control
procedure (SQC) is applied to detect and correct shifts. Since SQC can be planned to trap shifts of different sizes, the threshold set for SQC determines
the defect rate expected for long-term.

Gene therapy with adeno-associated virus vector 5–human factor IX in adults with hemophilia B

Gene therapy for hemophilia B aims to ameliorate bleeding risk and provide endogenous
factor IX (FIX) activity/synthesis through a single treatment, eliminating the requirement
for FIX concentrate. AMT-060 combines an adeno-associated virus-5 (AAV5) vector with a
liver-specific promoter driving expression of a codon-optimized wild-type human FIX
gene. This multinational, open-label study included 10 adults with hemophilia B (FIX £2%
of normal) and severe-bleeding phenotype.

IgM Augments Complement Bactericidal Activity with Serum from a Patient with a Novel CD79a Mutation

Antibody replacement therapy for patients with antibody deficiencies contains only IgG. As a result, concurrent IgM
and IgA deficiency present in a large proportion of antibody deficient patients persists. Especially patients with IgM
deficiency remain at risk for recurrent infections of the gastrointestinal and respiratory tract. The lack of IgM in the
current IgG replacement therapy is likely to contribute to the persistence of these mucosal infections because this
antibody class is especially important for complement activation on the mucosal surface. We evaluated whether
supplementation with IgM increased serum bactericidal capacity in vitro. Serum was collected from a patient with
agammaglobulinemia and supplemented with purified serum IgM to normal levels. Antibody and complement deposition
on the bacterial surface was determined by multi-color flow cytometry. Bacterial survival in serum was
determined by colony-forming unit counts. We present a patient previously diagnosed with agammaglobulinemia
due to CD79A (Igα) deficiency revealing a novel pathogenic insertion variant in the CD79a gene
(NM_001783.3:c.353_354insT). Despite IgG replacement therapy and antibiotic prophylaxis, this patient developed
a Campylobacter jejuni spondylodiscitis of lumbar vertebrae L4–L5. We found that serum IgM significantly contributes
to complement activation on the bacterial surface of C. jejuni. Furthermore, supplementation of serum IgM
augmented serum bactericidal activity significantly. In conclusion, supplementation of intravenous IgG replacement
therapy with IgM may potentially offer greater protection against bacterial infections, also in the context of increasing
antibiotic resistance

Febrile Urinary Tract Infections in Children

Antibiotic treatment of children with febrile urinary tract infections has almost
eliminated the risk of death, which was approximately 20% among children hospitalized
for acute pyelonephritis in the early 20th century. Some 50 years ago, one
study described renal parenchymal injury in 210 of 597 children treated for recurrent
urinary tract infections. Another study in that era reported on an 11-to-27-year
follow-up of 72 children hospitalized for urinary tract infections; 18% had died, 8%
had progressive renal insufficiency, and 22% had persistent untreated or recurrent
infection. Both studies assumed that kidney damage was related solely to urinary
tract infection, overlooking the possibility that congenital renal abnormalities contributed
to these outcomes. In the early 1970s, the evolving concept of reflux nephropathy
linked vesicoureteral reflux to pyelonephritis and late renal scarring.
Consequently, children who had had febrile urinary tract infections were routinely
evaluated for urinary tract abnormalities and often received long-term antibiotic prophylaxis10,;
surgical correction of vesicoureteral reflux became standard care.

Community-Acquired Pneumonia in Children: the Challenges of Microbiological Diagnosis

Red cell exchange transfusions lower cerebral blood flow and oxygen extraction fraction in pediatric sickle cell anemia

Blood transfusions are the mainstay of stroke prevention in pediatric sickle cell anemia
(SCA), but the physiology conferring this benefit is unclear. Cerebral blood flow (CBF) and
oxygen extraction fraction (OEF) are elevated in SCA, likely compensating for reduced
arterial oxygen content (CaO2). We hypothesized that exchange transfusions would decrease
CBF and OEF by increasing CaO2, thereby relieving cerebral oxygen metabolic
stress. Twenty-one children with SCA receiving chronic transfusion therapy (CTT) underwent
magnetic resonance imaging before and after exchange transfusions. Arterial spin
labeling and asymmetric spin echo sequences measured CBF and OEF, respectively, which
were compared pre- and posttransfusion. Volumes of tissue with OEF above successive
thresholds (36%, 38%, and 40%), as a metric of regional metabolic stress, were compared
pre- and posttransfusion. Transfusions increased hemoglobin (Hb; from 9.1 to 10.3 g/dL;
P < .001) and decreased Hb S (from 39.7% to 24.3%; P < .001). Transfusions reduced CBF
(from 88 to 82.4 mL/100 g per minute; P 5 .004) and OEF (from 34.4% to 31.2%; P < .001).
At all thresholds, transfusions reduced the volume of peak OEF found in the deep white
matter, a location at high infarct risk in SCA (P < .001). Reduction of elevated CBF and OEF,
both globally and regionally, suggests that CTT mitigates infarct risk in pediatric SCA by relieving cerebral metabolic
stress at patient- and tissue-specific levels.

Zoonotic Transfer of Clostridium difficile Harboring Antimicrobial Resistance between Farm Animals and Humans

The emergence of Clostridium difficile as a significant human diarrheal
pathogen is associated with the production of highly transmissible spores and the
acquisition of antimicrobial resistance genes (ARGs) and virulence factors. Unlike the
hospital-associated C. difficile RT027 lineage, the community-associated C. difficile
RT078 lineage is isolated from both humans and farm animals; however, the geographical
population structure and transmission networks remain unknown. Here, we
applied whole-genome phylogenetic analysis of 248 C. difficile RT078 strains from
22 countries. Our results demonstrate limited geographical clustering for C. difficile
RT078 and extensive coclustering of human and animal strains, thereby revealing a
highly linked intercontinental transmission network between humans and animals.
Comparative whole-genome analysis reveals indistinguishable accessory genomes
between human and animal strains and a variety of antimicrobial resistance genes in
the pangenome of C. difficile RT078. Thus, bidirectional spread of C. difficile RT078
between farm animals and humans may represent an unappreciated route disseminating
antimicrobial resistance genes between humans and animals. These results
highlight the importance of the “One Health” concept to monitor infectious disease
emergence and the dissemination of antimicrobial resistance genes.

Comparison of Four Serological Methods and Two Reverse Transcription-PCR Assays for Diagnosis and Surveillance of Zika Virus Infection

T Zika virus (ZIKV) is a mosquito-borne flavivirus that is responsible for recent
explosive epidemics in the Americas. Notably, ZIKV infection during pregnancy
has been found to cause congenital birth defects, including microcephaly, and ZIKV
has been associated with Guillain-Barré syndrome in adults. Diagnosis and surveillance
of Zika in the Americas have been challenging due to similar clinical manifestations
and extensive antibody cross-reactivity with endemic flaviviral diseases, such
as dengue. We evaluated four serological and two reverse transcription-PCR (RT-PCR)
methods in acute-phase (mean day, 1.8), early-convalescent-phase (mean day, 16.7),
and late-convalescent-phase (mean, ~7 months) samples from the same individuals
in a long-term pediatric cohort study in Nicaragua. Well-characterized samples from
301 cases of Zika, dengue, or non-Zika, nondengue febrile illnesses were tested.
Compared to a composite reference, an in-house IgM antibody capture enzymelinked
immunosorbent assay (MAC-ELISA) and the NIAID-Biodefense and Emerging
Infections (BEI) MAC-ELISA measuring IgM yielded sensitivities of 94.5% and
70.1% and specificities of 85.6% and 82.8%, respectively. The NS1 blockade-ofbinding
ELISA measuring anti-ZIKV NS1 antibody levels yielded sensitivities of 85.0%
and 96.5% and specificities of 91.4% and 92.6% at early and late convalescence, respectively.
An inhibition ELISA detecting total anti-ZIKV antibodies had sensitivity
and specificity values of 68.3% and 58.3% for diagnosis and 94.0% and 98.6% for
measuring annual infection incidence. Finally, the ZCD and Trioplex real-time RT-PCR
assays detecting Zika, chikungunya, and dengue viruses both yielded a sensitivity of
96.1% and specificity of 100%. Together, these assays resolve the urgent need for diagnostic
and surveillance tools for countries affected by Zika virus infections

Immunodeficiency in Bloom’s Syndrome

t Bloom’s syndrome (BS) is an autosomal recessive
disease, caused by mutations in the BLM gene. This gene
codes for BLM protein, which is a helicase involved in
DNA repair. DNA repair is especially important for the development
and maturation of the T and B cells. Since BLM is
involved in DNA repair, we aimed to study if BLM deficiency
affects T and B cell development and especially somatic
hypermutation (SHM) and class switch recombination
(CSR) processes. Clinical data of six BS patients was collected,
and immunoglobulin serum levels were measured at different
time points. In addition, we performed immune phenotyping
of the B and T cells and analyzed the SHM and CSR in
detail by analyzing IGHA and IGHG transcripts using nextgeneration
sequencing. The serum immunoglobulin levels
were relatively low, and patients had an increased number of
infections. The absolute number of T, B, and NK cells were
low but still in the normal range. Remarkably, all BS patients
studied had a high percentage (20–80%) of CD4+ and CD8+
effector memory T cells. The process of SHM seems normal;
however, the Ig subclass distribution was not normal, since the
BS patients had more IGHG1 and IGHG3 transcripts. In conclusion,
BS patients have low number of lymphocytes, but the
immunodeficiency seems relatively mild since they have no
severe or opportunistic infections. Most changes in the B cell
development were seen in the CSR process; however, further
studies are necessary to elucidate the exact role of BLM in
CSR

Immunologists are increasingly being asked to assess patients with non-classical and secondary antibody deficiency
to determine their potential need for immunoglobulin replacement therapy (IGRT). Immunoglobulin is a limited, expensive
resource and no clear guidance exists for this broad patient group. The purpose of this survey is to establish what factors influence
the decision to commence IGRT in adult patients, when diagnostic criteria for primary antibody deficiency are not fulfilled.

Genotyping Multidrug-Resistant Mycobacterium tuberculosis from Primary Sputum and Decontaminated Sediment with an Integrated Microfluidic Amplification Microarray Test

There is a growing awareness that molecular diagnostics for detect-totreat
applications will soon need a highly multiplexed mutation detection and identification
capability. In this study, we converted an open-amplicon microarray hybridization
test for multidrug-resistant (MDR) Mycobacterium tuberculosis into an entirely
closed-amplicon consumable (an amplification microarray) and evaluated its performance
with matched sputum and sediment extracts. Reproducible genotyping (the
limit of detection) was achieved with 25 M. tuberculosis genomes (100 fg of M. tuberculosis
DNA) per reaction; the estimated shelf life of the test was at least 18
months when it was stored at 4°C. The test detected M. tuberculosis in 99.1% of sputum
extracts and 100% of sediment extracts and showed 100% concordance with
the results of real-time PCR. The levels of concordance between M. tuberculosis and
resistance-associated gene detection were 99.1% and 98.4% for sputum and sediment
extracts, respectively. Genotyping results were 100% concordant between sputum
and sediment extracts. Relative to the results of culture-based drug susceptibility
testing, the test was 97.1% specific and 75.0% sensitive for the detection of
rifampin resistance in both sputum and sediment extracts. The specificity for the detection
of isoniazid (INH) resistance was 98.4% and 96.8% for sputum and sediment
extracts, respectively, and the sensitivity for the detection of INH resistance was
63.6%. The amplification microarray reported the correct genotype for all discordant
phenotype/genotype results. On the basis of these data, primary sputum may be
considered a preferred specimen for the test. The amplification microarray design,
shelf life, and analytical performance metrics are well aligned with consensus product
profiles for next-generation drug-resistant M. tuberculosis diagnostics and represent
a significant ease-of-use advantage over other hybridization-based tests for diagnosing
MDR tuberculosis
